    hope you all are having a great weekend I am spending december revamping/organizing! I need a new intake form for owners/nonowner surrenders since that is what I mainly take !

    We take in dogs and cats but our cat area needs work! 

    Also if you have intake medical sheets you fill out and protocol give it all HAHA!!

    Hey Rachel!

    I attached our surrender forms for both dogs and cats, as well as our medical exam intake form. I find that people surrendering appreciate the thoroughness of it, and I like to take notes as I am talking to people during the surrender.

    We do something similar on our end and it really helps keep things organized. Having both the surrender form and the medical intake in one place makes the handoff smoother and you catch fewer surprises later. People usually open up more when you're writing things down as they talk. It slows the moment down and you get better details about the pet's routine and behavior.
