Two documents from Lifeline Animal Project:

• Heartworm Treatment Information for Canines - the heartworm treatment protocol we utilize for our dogs going through heartworm treatment. When our foster team speaks with the foster we go into detail as to the requirements from the foster needed to complete the heartworm treatment process and what information to pass along to the adopters. Our adoptions team also relays the details or stage of treatment the pet is in to the adopter to ensure the adopter understands as well.
• Medications/Instructions tracking form - The second document is a form we use when placing all of our animals on medications. We send a copy with the foster and keep the original so that our teams can keep a general track of when the medication should be finished. We are able to treat in house for heartworms so we also pre schedule their heartworm injections based on the estimated time the pet should be finished with meds.
