I agree that the transformation isn't immediate. I got my ESA letter almost two years ago. I got my dog as a puppy, so that stage was a bit stressful. But now we've bonded and he goes everywhere with me. I've decreased my medications (I've been on some for 30 years) in half! I don't know if I can directly attribute that to my dog, or whether it's because I've made multiple lifestyle changes. Chicken/egg argument. However, I had to get out of bed every morning. I had to go outside many times each day. I had to care for something outside of myself. I had to train and feed. All those things gave me structure and routine- something that was missing in my life.
In Tennessee, ESA don't get the same treatment as service animals. They aren't allowed in stores or restaurants. Apartments can decline to rent to me or can charge additional fees. I have to pay enormous pet fees on airlines and in hotels. Even on a limited income, this is 100% worth it.
