Sure! We have a full foster handbook (I've attached this) that gets sent out after foster training is complete. We require fosters to fill out the general volunteer application and attend general orientation, then they fill out a foster application and attend our prerecorded foster training (which has a quiz at the end so we know they watched the full training and heard the most important parts of training). The foster application and agreement is the one I attached already. We also have a form labeled, "foster pet guidelines",
https://hstt.org/sites/default/files/Foster%20Pet%20Guidelines_0.pdf, that gets signed before a pet goes into foster. We have each foster also sign a docusign form with the new foster pets name each time they foster.
Short of asking for a blood sample and promising us their first born...lol, we are located in CA, so it seems like a bit of an overboard process, however covering ourselves in everyway possible is important (depending on where you're located, you may not need all of the these types of forms).
We try to give our fosters as much information as possible so that they feel fully ready to foster and it limits the amount of cell phone calls/texts/emails we end up taking once they have a pet in their care.

GaDaH has finally found someone to coordinate a foster program.... yayyyyyy. I do have an old foster form, however, I saw in the library one that had been reviewed by attorneys and wanted to take a look.
