  • 1.  ACO/Police stray animal intake forms

    Hi everyone!
    I am the intake supervisor for my humane society. We are a non profit with municipal contracts. I'm looking to improve our stray animal intake process and was hoping to learn from other shelters. If you're willing to share, I'd love to see examples of your ACO/Police stray intake forms - specifically the ones that document whether an animal is stray vs. abandoned vs. seized, location found, officer information, and any related details.

    Our current form is pretty lackluster, and I'm hoping to make it more thorough and consistent for both intake and legal/medical clarity. Any examples, tips, or things you've found especially helpful on your forms would be greatly appreciated.
