    Hi all! I'm looking to create a jotform questionnaire for dogs adopted from my shelter that have been heartworm treated. We are fortunate enough to be able to treat dogs for heartworms here at my shelter at no additional cost to the adopters. I'm wanting to ask adopters a few follow-up questions that may help showcase our medical team, get positive community attention, and help us to get additional grants or funding for heartworm treatment services. So far I only have a few questions. I don't want it to be overwhelming for the adopters to complete but also want to be able to get helpful information. Any suggestions are appreciated!

    1. Would you have adopted a heartworm positive dog if treatment costs had not been covered by the shelter?
    2. Was your dog retested for heartworms after treatment?
    3. Was the heartworm test negative or positive?
    4. Do you feel you were given sufficient information about heartworm disease at the time of adoption?
    5. Did you feel you the heartworm treatment process was explained thoroughly at the time of adoption?
    6. Has your dog been to a veterinarian in the last 6 months to 1 year? (This is to help get an idea of how we have impacted private practices aka new clients)
