Animal Welfare Professionals

Our volunteer Board of Directors is seeking to add new members and it was suggested we have interested parties fill out an application.   Does anyone have a template I can use they would be willing to share?    Thanks for the help.

We are a volunteer run organization including our board of directors. The first few years our board was made up of like minded people who we knew would have the animals best interest at heart. Last year we decided, since we were growing,  we adopted a very simple application. Our board also put together additional information to share with potential board members including a board member readiness questionarre, a "job" description list and a board member agreement.  I've attached these pages here.
