Hi Amanda! We're in year one of rolling out a new process. The end-of-year performance evaluation form we'll be using is based on the following template from The Management Center:

In order to prepare managers to be able to use that form, we've also created a monthly check-in form. The Management Center has its own monthly check in form, but it was too complicated for the majority of our staff. Instead we're using one of our own creation that simply states the goals and competencies staff are supposed to be working on, along with the underlying core values of our organization, and tracks progress on such throughout the year. It also has room for discussion points. Being able to review this monthly data will make end-of-year evaluations much more efficient and fair.
