My shelter has a partnership with a nearby correctional facility, where we send a pack of dogs every two months to get basic obedience training. We have recently expanded the program to have a separate pack on their own timeline, who have more intense behavioral issues, such as dog aggression, dog reactivity, and fear aggression. We have a lot more information on these dogs than our normal pack of dogs. Does anyone have a training intake form they use? I'm looking for a form that asks all questions a shelter may know about the dog to then be put into training. It would be helpful if the form was specific for behavioral challenged dogs. I'm thinking this may be from another shelter or maybe something you fill out for transfers. Thank you!
