Hi!

I'm looking to create a "report card" to send home with foster parents, specifically dog fosters,  for them to fill out and return for potential adopters to learn more about the dog. If anyone already has something like this created I'd love to get some ideas from you Thanks!

Hi  CCASFoster,

The amazing team at Pima Animal Care Center created this. Its for day trips and sleepovers, but you could use it for longer foster.  Do you use Maddie's Pet Assistant? We're also working on a survey for Maddie's Pet Assistant where foster caregivers enter info so that it's easier for you to write a bio... but it's not available yet.

I've attached a pdf of the day trip report card our Dog Foster Coordinator came up with for use on the kennels, and the template we are starting to use in Chameleon.

Hello! I am new to this forum and the shelter industry and I am so excited about the content!

I'm curious, what does your Google site look like on the back end? Does it input all the foster survey info into a report for you? It is a great site. :) 

We give this form to all of our fosters. We have had good success using the form to match animals with potential adopters and the fosters enjoy filling it out.
