I'm a volunteer with Refurbished Pets of Southern Michigan and we have a foster appliation on our website that we would like to redesign and are looking for tips on what we should include. New research states people are turned away by a ton of questions. Our foster application can be found here:

We only ask name, phone, address, age and ask them to give us a quick blurb on why they want to foster! It's automatically uploaded into AirTable, a really cool spreadsheet application, and our foster coordinator gets a notification. She personally calls each potential parent to get to know them and invite them to an orientation.
