Animal Welfare Professionals

We're going to be starting a follow up to adoptions.  Adopters will be phoned by a trained volunteer, and asked how things are going, etc.  I want to give the volunteer a list of things to ask and look for, along with set answers and things she needs to pass on to me.  Those who already do this - what works for you?  

I would suggest using Maddies Pet Assistant first and then only follow up calls on the ones marked in red.  We are getting great responses on the survey's and then can spend quality time on the ones that need more attention.  We are also getting feedback from our adopters that they find it very helpful.

I'd very much like to!  We're in the process of switching our database to PetPoint, and then we can use the Pet Assistant easier, but until then, having a volunteer follow up with phone calls is the best we can do.  Is there anywhere I can find the Pet Assistant questions?
