We are in the process of a revamp of all of our internal forms and I was curious as to what other organizations use to track vaccines, treatments given, etc while the animal is still in shelter. We enter everything into petpoint but we still keep an internal log as well. Our current form tracks vaccines given, dewormers, tests, and flea medications, but I was hoping to incorporate recording three microchip scans, shave for spay scar, and other more intake exam type information. I have had a lot of success finding examples of the other forms I have updated online with the ASPCA Pro, HSUS, ect. but for some reason the only examples I can find of vaccine history is the booklets given out by veterinarians.  Thanks!

Why do you keep a paper copy in addition to the electronic record?  Unless that's a requirement it seems like double the work.

We do both as well and keep both because of human error. Things can get entered incorrectly, not entered at all, etc.

We use PetPoint also.  We created a template in the vaccination area.  Called it our "intake" exam.  Put in all those checkoff things.  So when we do the exam, we just go in to the template and fill in the blanks.  Also have an 'out take exam template".  Can do a template for anything.  Feel free to contact me separately if you want a better idea of how we use or created this.

Would you be willing to share an example of your "intake" and "out take" exam templates?  We are working on redoing many of our forms as well this would be very helpful.  We don't currently have a good shelter management software package and are evaluating options for this as well.  Thanks!

Amanda, in PetPoint under the animal identification tab there is a spot to record the microchip scans. Also under exam the treatment tab you can create a treatment type of "tasks" and then treatment type "Shave for spay scar", etc. I hope this helps.
