Animal Welfare Professionals

Does anyone have a survey they have used and wouldn't mind sharing that you have sent to foster parents (or volunteers) to find out why they may no longer foster?  I found the FAST 15 survey, but it's really only for people that have fostered in the past 12 months.  I'm looking for something for those that have been inactive.  Thanks!

These survey results may or may not be helpful. One is for caregivers and one is for foster coordinators.
