When I used to work as a gift processor for a university, we would also ask the donor's preference (monthly vs. year-end receipts). The link below might be helpful for what to include, and I also recommend your organization's EIN (although not required, as mentioned in the article). I would also note the importance of #5.

Hello everyone. We currently have 5 people who donate monthly and for each one I send out a general thank you every month that includes a blurb about taxes.
My question is: after the new year (or fiscal new year) do you send a statement showing the total amount a donor has donated, that they can use for their tax return? If so, is there a template somewhere?
